For your convenience, the link to the I-693 paper form that you need to bring to your visit is provided below:
Cost of immigration exam: $275 (this includes the evaluation and physical examination by Dr. Sapna Rajput, order and review of labs and immunizations records, completed I-693 form in a sealed envelope).
Please note that the immigration exam with Dr. Rajput is not covered by insurance.
Blood test and Vaccination information:

Blood Test:
You can go to any lab that accepts your insurance, such as Quest, LabCorp, Kaiser, PAMF, or Stanford. Coverage depends on your specific plan. Dr. Rajput offers discounted lab pricing through Quest. If you don’t have insurance, Quest and LabCorp are good options and are located near our clinic. Dr. Rajput will order the necessary labs for you. -
Vaccinations:
You can get vaccinations at any pharmacy using your insurance. Coverage depends on your plan. If you don’t have insurance, you can still receive vaccinations at any pharmacy.
Documents you need to bring to your visit:
-I-693 form with pages 1-3 filled out where applicable and your name filled out on the top of every page.
-Original and hard copy of government-issued driver’s license or passport
-Hard copy of immunization records in English
-Hard copy of Tb test and chest xray results (only if Tb positive in past)
Length of time for I-693 packet to be completed:
Generally, the packet will be ready in 7-10 business days from the day of visit. The blood work results typically take 3-5 days.
